Job description

We are seeking a full-stack Software Engineer to join the team behind PRVNT, Nsight Health's enterprise care management platform serving 350+ clinics and real Medicare patients across RPM, CCM, PCM, and BHI programs. This is a production environment where the work matters and the stakes are real.

Reporting to the VP of Engineering, you will own full-stack development across clinical workflows, backend services, EHR integrations, and HIPAA-compliant data handling. You are not a specialist waiting for clean tickets-you move across the stack, close gaps, and ship features that work in a regulated environment. Development is moving fully in-house, vendor dependencies are being replaced with owned infrastructure, and AI is not a feature here; it is the architecture. The engineers we are hiring now are the ones who make that possible.

AI Fluency Requirement - Non-Negotiable

Nsight Health is an AI-first organization. Every member of our leadership and operations team is expected to actively use AI tools in their day-to-day work - not as a novelty, but as a core productivity multiplier. This role requires genuine curiosity about AI, comfort experimenting with tools like Claude, ChatGPT, and workflow automation platforms, and the judgment to know when AI helps and when it doesn't. If AI makes you uncomfortable, this is not the right role.

Key Responsibilities

Clinical Platform Development: Maintain and extend core PRVNT features including patient chart, clinical dashboards, adherence workflows, billing ledger, SOAP notes, and assessment delivery. Extend clinician and manager dashboards to surface actionable data without requiring staff to hunt for it. Support automated delivery of clinical assessments such as PHQ-9 and GAD-7 via secure SMS.

Backend & Integrations: Integrate with EHRs using HL7 FHIR and SFTP-based connections. Improve async reliability across messaging, letters, and billing file generation through job queuing and retry logic. Maintain Twilio short code messaging pipelines and extend billing capabilities including trending reports and CPT code attribution.

Access, Permissions & Compliance: Implement and maintain nurse license validation including state-level checks, expiration tracking, and care access restrictions. Ensure HIPAA-compliant data handling across all features and BAA-covered third-party integrations.

AI-Native Development: Use Claude Code and GitHub Copilot as active development partners daily. Apply AI tooling to accelerate feature delivery, test generation, and documentation. Track and improve AI output ratios as a measurable productivity metric.

Compliance by Design: By building HIPAA compliance into data handling and access controls from the first commit, you protect both patients and the organization.

Requirements

Do you have experience in TypeScript?, * 3+ years of full-stack software engineering in production environments with real features shipped in real codebases

  • Frontend proficiency in React and TypeScript
  • Backend proficiency in Node.js, Python, or comparable; ability to build APIs, services, and integrations without a scaffold
  • Experience with async job queuing and event-driven architectures (Redis, BullMQ, SQS, or similar)
  • Comfort working in a HIPAA-regulated environment with actual working knowledge of what it requires at the data and access layers
  • Daily, demonstrated use of Claude Code, GitHub Copilot, or equivalent AI-assisted development tools
  • Strong written communication; ability to document decisions and surface gaps in real time

Preferred:

  • Familiarity with EHR integration patterns, HL7 FHIR, or SFTP-based data exchange
  • Experience in healthcare or another regulated industry
