Job description

  • Assist individuals in crisis prevention and stabilization to increase the likelihood that the individual can remain in his/her own community.
  • Propose a service plan within the stated timeline and thereafter continually monitor a plan which will delineate individual service needs and a plan to access the services. For IDD clients, efforts will be made to prevent or delay institutional placement and to enhance inclusion in the community.
  • Provide transportation for clients as needed and directed by supervisor.
  • Serve as a link between individuals, their families, Life Help services and the community.
  • Provide individuals and their families with information and education concerning diagnosis, medication and A&D issues.
  • Conduct home visits and assist the consumer in availing themselves of needed community services.
  • Work with the family or other responsible parties of each consumer enrolled.
  • Submit weekly billing slips to the CSS Quality Assurance for SED clients.
  • Submit all monthly reports required by the Department of Mental Health and Life Help.
  • Provide medication monitoring as needed and consult with appropriate medical staff.
  • Encourage individuals to attend all needed and appropriate appointments.

Requirements

  • Bachelor’s Degree in a related mental health field.
  • Valid driver’s license and reliable personal vehicle.
  • Case Management Certification from Mississippi Department of Mental Health.
