Job description
We are an early-stage health-tech startup looking for a Staff Full-Stack Software Engineer to help design, build, and own the core of our telehealth platform - from the async Python domain layer that encodes our business logic to the browser-based UI where clinicians see and examine patients. This is a hands-on, senior individual-contributor role: you will set technical direction and make architectural decisions while still writing and reviewing code every day. Our backend is the heart of the system, so this role leans backend, but you should be comfortable working across the full stack. As a small team, we all wear many hats, so responsibilities span the stack and shift as needed. You will be expected to use AI-based workflows and tools on a daily basis to accelerate your efforts and capabilities.
This is currently a full-time, hybrid position - a mix of in-office work at our Austin, TX office and remote.
Responsibilities
Backend & Domain Architecture
- Design and build the server that powers our telehealth platform: an async Python application (Tornado, asyncio) on a MariaDB database, backed by a domain-centric architecture where business logic lives in one authoritative, testable place and API handlers are thin adapters on top of it.
- Own core architectural decisions - data modeling, domain interfaces, authorization, transaction boundaries - and steward a mature codebase through its ongoing modernization to typed Pydantic models, aiomysql, and domain-layer authorization.
- Model complex clinical and operational domains (patients, providers, facilities, visits, appointments, subscriptions) as clear, correct, well-typed code.
- Write strong tests. We treat the domain layer as the canonical implementation and hold it to a correspondingly high bar for evidence of correctness.
Integrations, AI, & Data
- Build and maintain integrations with external Electronic Health Record (EHR) systems - such as Epic - exchanging clinical data through healthcare interoperability standards (e.g. HL7, FHIR).
- Build and improve our AI features.
- Integrate and maintain third-party services that make virtual care work: real-time video, messaging, payments, cloud infrastructure, and asynchronous processing.
- Work with clinical and operational data - reporting, exports, and document generation - with an eye toward accuracy and privacy.
Full-Stack Delivery & Real-Time Video
- Ship features end to end, including the browser client: a JavaScript (ES modules) single-page app, with real-time video visits, charting, and clinical workflows.
- Connect the frontend to the domain through clean API contracts, and make the experience fast and reliable for clinicians working in low-bandwidth, real-world settings.
Reliability, Security & Quality
- Keep a healthcare-grade system reliable, observable, and secure; treat patient data (PHI) with the care it demands.
- Raise the bar on code quality, testing, and engineering practice; participate in and lead code review.
Technical Leadership
- Set technical direction and make pragmatic architecture calls for a small team with a large surface area.
- Mentor teammates, share context, and multiply the output of everyone around you.
Other
- Collaborate on other large/small projects as needed.
- Other related tasks as needed.
Requirements
- Bachelor’s degree in Computer Science or a related technical field.
- 8-10+ years of professional software engineering experience building and operating production backend systems, with deep proficiency in Python, relational databases, and asynchronous programming patterns.
- Staff-level engineering judgment: you have designed system architecture, owned significant components end to end, and served as a technical anchor for a team.
- Comfortable across the full stack - able to build and debug browser-based UIs in JavaScript.
- Strong testing discipline and a bias toward correctness, clarity, and maintainable, declarative code.
- Experience integrating third-party APIs (e.g. video/WebRTC, payments, messaging) into a cohesive product.
- Comfortable working with LLMs / AI features in production.
- Highly organized, self-directed, and able to own priorities independently across a broad surface area.
- Adaptable and comfortable with ambiguity in a fast-moving, early-stage environment.
- High proficiency in using AI tools to accelerate work outcomes.
- Strong passion for creating a positive social impact through improved access to healthcare.
- Direct experience integrating with Electronic Health Record (EHR) systems and healthcare interoperability standards such as HL7 and FHIR is a significant plus.
- Previous experience in healthcare/health-tech, or working with sensitive data under HIPAA, is a strong plus., * How many years have you built and operated production backend systems in Python (or a comparable language)?
