Job description

Field Service Supervisor (FSS) is responsible for facilitating and overseeing the Direct Care Workers (DCW) who provide direct care services for our clients. FSS ensured that DCWs are trained on the client Plan of Care (POC) providing high quality and efficient health care and supportive services. FSS will provide self-management goals and referrals to both internal and external resources. Responsible for the training, supporting and the development of all FSS(s). The Field Supervisor is responsible to ensure that they review the EVV of DCWs to ensure that the tasks are complete and to monitor the clock in and clocking out of shifts. Primary Job Responsibilities:

  • Assist with monthly Client QA Visits and completion of Client Survey on caseload
  • Updating current DCW schedules with Home Care Clients
  • Client Wellness Check-ups - The clients assigned DFO will organize all Wellness Check-ups visits. The Director may also request FSSs to conduct Wellness Check-ups visits
  • Filling & Covering Shifts - FSSs must find coverage or provide coverage themselves
  • Tracking Overtime usage and working with Payroll Specialist with approving overtime
  • Overseeing and tracking client referrals and onboarding Potential Clients
  • Overseeing the on-call phone and assigning coverage for the on-call phone as needed
  • Updating current DCW schedules as needed
  • Assist with conducting Intake with New clients
  • Identify and link patients with community resources to facilitate referrals and respond to social service needs
  • Assist with On-Call phone rotation

Requirements

  • Strong written and verbal communication skills
  • Teamwork orientation
  • Able to take and follow through with delegated tasks
  • Proven data analysis skills, organizational skills, and resourcefulness in problem solving
  • Intermediate proficiency in Microsoft applications: Excel, Word, Outlook, PowerPoint
  • Strong attention to detail and follow up
  • Confidentiality is of the upmost importance and must be able to maintain client privacy per HIPAA laws
