Job description

We are looking for an Epic Healthy Planet Consultant with hands-on experience in Risk Roster and Attribution & Assignment (A&A). The ideal candidate must be Epic Healthy Planet Certified and have experience supporting Population Health, Value-Based Care, and Epic Healthy Planet implementations.
