Job description

Plans by Amber LLC is a Support Planning company looking for a new Support Planner to support people choosing to utilize CDCS through their waiver. Waivers that are currently accepted are: CADI, DD, AC, EW, and CAC. Support Planners are responsible for meeting with clients and their team, completing CDCS paperwork, and determining what is available through the waiver and explaining those goods/services to the client so that they can make an informed decision on what they want to utilize through their CDCS plan.

You have the ability to accept or deny any clients offered. You will work at times that work with your schedule while still meeting county expectations and company turnaround time expectations. You can start with a small caseload if you would like, otherwise, your caseload will build slowly as new clients choose Plans by Amber LLC. You will be an Independent Contractor.

Duties

  • Hold intake and annual meetings (typically via phone/video)
  • Complete required paperwork (CSP, HSP, and Annual Signature Pages)
  • Communicate with Case Managers, Guardians/Parents, FMS agencies, and providers
  • Determine services/goods that would benefit person and explain them to person
  • Help person monitor their support plan
  • Help person solve problems that may occur
  • Initial client calls within 1-2 days after accepting the client
  • Schedule the intake within 1 week of accepting the client
  • Schedule and hold renewal meetings 2 months before renewal
  • Turn in all paperwork 1 month+ before renewal date

Requirements

  • Current or previous Waiver Case Manger (CM) or Waiver CM Supervisor with CDCS experience as the CM/CM Supervisor
  • Experience with CDCS, professionally
  • Active Certified Support Planner Certification or ability to obtain it before starting
  • Proficiency in English, both written and verbal, is essential.
  • Knowledgeable on required documentation needed for CDCS and able to complete CSP and HSP
  • Knowledgeable on what is available through CDCS and the requirements
  • Experience with Microsoft Word and Adobe.
  • Experience in support planning is a plus.
