Job description

The Transition Manager provides strategic leadership, oversight, and direction for all transition services across the Intermediate Unit. This Act 93 position is responsible for the development, implementation, and continuous improvement of transition programming designed to prepare students with disabilities for post-secondary education, employment, and independent living.

The Transition Manager will direct transition staff, ensures compliance with federal and Pennsylvania Department of Education regulations, and leads coordinated efforts with internal and external stakeholders to deliver high-quality, outcome-driven transition services., * Oversee the development and implementation of individualized transition plans aligned with student IEPs and post-secondary goals.

  • Coordinate and oversee scheduling of transition activities, including staff assignments, student placements, and program logistics.
  • Ensure effective coordination of transportation and coverage for field trips and CBI experiences.
  • Collaborate with educational teams to align curriculum, instruction, and transition outcomes, specifically working on a consistent basis with the Transition TaC member.
  • Conducts regular meetings (minimum of once per quarter) with teachers of transition-aged students to discuss and plan for individual student needs.
  • Attend the monthly Transition Counsel meetings held at the Intermediate Unit.
  • Monitor timelines, documentation, and compliance requirements related to transition services and reporting.
  • Prepare reports, analyze data, and present program outcomes to leadership.
  • Serve as the primary liaison between the Intermediate Unit and community partners, including vocational rehabilitation and adult service providers.
  • Develop, coordinate, expand, and sustain partnerships with local businesses, agencies, and post-secondary institutions to support student transition outcomes.
  • Facilitate job shadowing, internships, and employment opportunities for students.
  • Plan, organize, and host a minimum of three (3) career fairs annually at New Horizon, New Horizon North, and the Intermediate Unit to connect students with employment and training opportunities.
  • Lead and direct transition staff, including coordinators, job coaches, and support personnel.
  • Develop and implement a comprehensive transition services framework aligned with PDE requirements, and IEP transition planning compliance.
  • Provide direction and structure to ensure consistent, high-quality programming across all IU buildings and programs.
  • Utilize process mapping and continuous improvement strategies to enhance efficiency, compliance, and service delivery.
  • Establish systems for data collection, progress monitoring, and program evaluation.
  • Demonstrate exceptional attention to detail in managing complex logistics and compliance documentation.
  • Carry out additional duties as assigned by the Executive Director, Director of Special Education, or Supervisor.

Requirements

Do you have a valid Driver’s License license?, Do you have experience in Teamwork?, Do you have a Master’s degree?, * Degree in Special Education, or related area required.

  • Masters degree in special education, preferred.
  • Minimum of 5 years of satisfactory teaching experience, required.
  • Transition experience preferred.
  • Demonstrated expertise in process mapping, systems development, and program structure.
  • Exceptional attention to detail and accuracy in compliance and documentation.
  • Strong leadership and staff supervision skills.
  • Strong organizational skills with the ability to manage complex systems and multiple priorities.
  • Ability to build and sustain meaningful community partnerships.
  • Excellent communication and collaboration skills across diverse stakeholders.
  • Ability to design and deliver effective training and professional development.
  • Knowledge of transition regulations, IEP processes, and post-secondary outcomes.
  • Valid Driver’s License required.
  • Ability to provide clearances as required by the State Departments in Pennsylvania
