Job description
We are seeking a dynamic and compassionate Bi-Lingual Care Coordinator to join our dedicated in-home healthcare team on a full-time basis. In this vital role, you will serve as a primary point of contact for diverse senior populations, providing culturally sensitive support and coordination. Your expertise will ensure seamless communication, discharge planning, and case management, all while promoting client-centered care. - Ensure the service plan for the client is accurately documented, and ADLs are
- completed accurately.
- Ensure all visits are verified on a timely basis.
- Conduct intake assessment, monthly wellness check, and supervisory visits for
- clients
- Ensure all monthly authorized hours for the client are fully utilized.
- Check in on the client regularly to evaluate and document their progress and
- satisfaction with the services provided.
- Educate clients about In-Home Service job functions, client and HCA rights and
- responsibilities.
- Handling patient case management completely and effectively
- Maintaining patient confidentiality and privacy.
- Take initiative in resolving conflicts between clients and caregivers.
- Ensure all clients are staffed promptly.
HCA Management/ Training:
- Recruitment of home care aides and ensuring assigned training is completed before they are assigned to clients.
- Creating schedules to align with clients’ ADLs and IADLs
- Training Home Care Aide at the time of hire and quarterly as required by the Illinois Department of Public Health and other government agencies.
- Ensure all Authorized hours for clients are scheduled and completed.
- Attend ongoing training and courses to keep abreast of new developments in healthcare.
- Ensure background checks are completed at the time of hire and annually.
Duties
- Act as a bilingual liaison to facilitate effective communication between patients, families, and healthcare providers, ensuring clarity and understanding across language barriers
- Coordinate patient care activities, including intake assessments, discharge planning, and social services referrals, to optimize health outcomes
- Assist with utilization review and management processes by reviewing medical records and ensuring appropriate resource utilization in compliance with HIPAA regulations
- Collaborate with hospital teams such as nursing, social work, behavioral health, and medical staff to develop comprehensive care plans tailored to individual needs
- Maintain accurate documentation within EMR (Electronic Medical Record) and EHR (Electronic Health Record) systems, ensuring data integrity and confidentiality
- Support patients through complex healthcare processes by providing education on medical terminology, health insurance options, and managed care plans
- Participate in case management activities for diverse populations, including pediatrics, ICU patients, trauma center cases, and hospice care recipients
Requirements
- A care coordinator should exhibit superb communication skills.
- Collaborate with clients to determine needs, modify care plan, and evaluate interventions.
- Care coordinators possess the leadership qualities needed to guide individuals of varying backgrounds towards a common goal for the betterment of a patient’s health and quality of life.
- Treat clients with empathy and respect, and conduct oneself professionally., * High School Diploma or Equivalent GED
- Proficiency in Microsoft Office applications, along with EMR/EHR systems, for efficient record keeping
- Bilingual fluency in English and another language is essential; excellent client communication skills are required to serve diverse cultural backgrounds effectively
- Knowledge of HIPAA regulations to ensure patient privacy and data security at all times
- Ability to handle intake procedures efficiently while demonstrating empathy and professionalism in high-pressure situations.
