PHP/IOP Therapist - Dual Diagnosis 1pm - 9pm

• Facilitate individual counseling sessions
• Lead psychoeducational and process therapy groups
• Conduct family therapy sessions
• Complete biopsychosocial assessments and treatment planning
• Develop individualized treatment plans
• Maintain accurate and timely EMR documentation
• Participate in multidisciplinary treatment team meetings
• Coordinate care with nursing, medical, case management, and recovery support staff
• Help patients develop healthy coping skills, relapse prevention plans, and long-term recovery goals
• Deliver compassionate, patient-centered care while maintaining clinical excellence
