Job description

The Navigation & Data Coordinator manages FCC referral workflows and data for the West Region, ensuring that families are contacted and connected to appropriate services while maintaining accurate, HIPAA-compliant documentation and supporting data-driven program management.

Essential Functions and Responsibilities:

  • Receive and monitor referrals from FCC nurses, hospitals, and partner agencies for West Region families.
  • Coordinate timely outreach and follow-up with families in collaboration with Community Outreach Specialists and other staff.
  • Maintain and regularly update the West Region resource directory, including maternal/infant health, behavioral health, housing, legal, and basic needs providers.
  • Enter and track referral, linkage, and outcome data in Unite Us and UCAN’s electronic health record (Evolv) or other approved systems.
  • Develop and run routine reports and dashboards for internal use and for CDPH reporting, including disaggregated equity indicators.
  • Coordinate and document distribution of infant essentials and safe sleep materials, including tracking both purchased and donated items.
  • Participate in FCC case conferences, partner meetings, and CQI activities as requested.
  • Ensure compliance with data privacy, security, and confidentiality policies.

Requirements

  • Bachelor’s degree in Social Work, Public Health, Data/Information Systems, or related field, or equivalent experience.
  • Experience in case management, care coordination, program coordination, or data management in a health or human-services setting.
  • Strong skills with databases, spreadsheets, and reporting tools; experience with electronic health records preferred.
  • Understanding of HIPAA, confidentiality, and ethical handling of sensitive information.
  • Detail-oriented, organized, and able to manage multiple priorities and deadlines.
  • Strong communication skills and ability to work collaboratively with internal and external partners.
