The TCM Provider Network Manager is responsible for maintenance of the TCM provider network through effective contract monitoring, relationship development, and provider support. This role collaborates with Advance Medical Home Plus (AMH+) and Care Management Agency (CMA) providers to ensure that Trillium meets all required benchmarks. The TCM Provider Network Manager is responsible provider oversight and compliance, including meeting facilitation, technical assistance, and monitoring. The TCM Provider Network Manager supervises a team of TCM Coordinators and develops processes and protocols to ensure uniformity in supporting, monitoring, and ensuring contract compliance of all certified and contracted TCM Providers. On a typical day, you might:

  • Lead and manage TCM Coordinators to facilitate effective management and support of provider-based TCM. Drive the development and maintenance of a robust provider-based TCM network by supporting the recruitment and development of Care Management Agencies (CMAs) and Advanced Medical Homes (AMH+s).

  • Build relationships with TCM providers to assess delivery of TCM, guide successful implementation, and provide technical assistance to ensure compliance with TCM requirements.
  • Collaborate with providers to identify and implement process improvements and best practices to ensure high-quality care.
  • Effectively facilitate project meetings with providers and staff to provide technical assistance, continually assess provider capacity and determine readiness to provide TCM.
  • Participate in TCM Readiness Reviews for certified providers as part of a cross-functional team, as required by contract.
  • Participate in and lead training meetings, conference calls, and web-based programs to stay updated on TCM requirements and guidelines.

Requirements

Do you have a valid Driver’s License license?, Do you have experience in Training delivery?, Do you have a Master’s degree?, * Associate’s degree and three (3) years of experience in network management, contracting, data management, practice management, or related healthcare operations; OR

  • Bachelor’s degree and one (1) year of experience in network management, contracting, data management, practice management, or related healthcare operations; OR
  • Equivalent combination of education/experience.
  • Must have a valid driver’s license.
  • Must reside within North Carolina.
  • Must be able to travel within catchment as required.

Preferred:

  • Master’s degree in a Human Services field.

  • Supervision of two (2) or more employees.
  • Fully licensed in the State of North Carolina as an LCSW, LCMHC, LMFT or LPA.
