Job description
We are seeking a compassionate and organized Transition Coordinator to support youth as they prepare to successfully transition from correctional facilities back into their communities. In this role, you will help coordinate education, employment, life skills, and reentry services while collaborating with schools, state agencies, community organizations, and families to promote positive long-term outcomes.
This position is ideal for professionals with experience in youth services, case management, customer service, education, social services, or community outreach who are passionate about making a meaningful impact.
Key Responsibilities
- Facilitate the Next Steps life skills curriculum for youth preparing for community reentry.
- Utilize the Transition Coordinator Handbook to ensure standardized transition practices across facilities.
- Complete and evaluate required transition consultations for each assigned student.
- Participate in weekly Administrative Release Committee (ARC) meetings to support and reinforce individualized reentry plans.
- Conduct follow-up phone calls with released youth at 1, 3, 6, 9, and 12 months after release.
- Represent the Indiana Department of Correction - Division of Youth Services at conferences, meetings, and community events.
- Build and maintain partnerships with organizations such as the Indiana Department of Workforce Development, HIRE, and Indiana University’s HOPE Mentoring program.
- Assist in developing individualized Education Learning Plans for youth upon facility entry.
- Coordinate with state, county, and private agencies to connect youth with community reentry resources.
- Help youth obtain essential identification documents, including Social Security cards, birth certificates, state IDs, and driver’s licenses.
- Support students transitioning back into public schools.
- Provide guidance on employment opportunities, apprenticeship programs, and military career pathways.
- Maintain accurate documentation and data entry related to youth services and program activities.
Requirements
- Strong written and verbal communication skills.
- Proficiency with Microsoft Office, including Word, Excel, Outlook, and PowerPoint.
- At least 1 year of customer service experience.
- At least 1 year of data entry experience.
- At least 1 year of experience working in a team-oriented environment.
Preferred Qualifications
- Experience working with at-risk youth.
- Experience supporting community-based or state-funded programs.
- Experience working in youth education, mentoring, or educational support programs.
- Background in case management, social services, workforce development, juvenile services, counseling support, or community outreach is a plus.

- Are you proficient in Microsoft Word, Excel, Outlook, and PowerPoint, and do you have at least 1 year of experience performing data entry and maintaining accurate records?
Benefits & conditions
$16.50 - $18.35 an hour - Contract
