The Case Manager & Direct Support Specialist provides both case management coordination and direct, person-centered support to individuals with intellectual and developmental disabilities. The position assists the QIDP and Division Director with coordinating, monitoring, documenting, and implementing services while also providing direct support to individuals in their homes and communities. The role promotes independence, choice, community inclusion, health, safety, and overall quality of life while helping ensure services are delivered in accordance with each individual’s person-centered plan and applicable organizational and regulatory requirements., * Provide direct care and support to individuals with disabilities in their homes, workplaces, and community settings.
- Assist with daily living activities, including personal hygiene, meal preparation, household chores, appointments, recreation, and community participation.
- Partner with individuals to support informed decisions and everyday choices related to finances, well-being, relationships, employment, and personal goals.
- Promote independence, self-direction, community integration, meaningful relationships, and skill development.

Case Manager & Direct Support Specialist
- Assist and support the QIDP in the development, understanding, implementation, and achievement of personal goals contained in individuals’ habilitation and person-centered plans.
- Assist the QIDP in coordinating effective communication among individuals, family members, guardians, Shared Living Providers, coworkers, healthcare professionals, Service Coordinators, and other members of the support team.
- Conduct home and community visits under QIDP supervision to observe the individual’s living environment, services, health and welfare, and progress toward goals.
- Identify and promptly report home deficiencies, Shared Living Provider concerns, behavioral changes, medication concerns, health and welfare issues, or other matters requiring follow-up.
- Assist with scheduling and attending Individualized Service Plan (ISP) meetings and other informational or treatment-related meetings to help ensure individual needs are addressed.
- Under QIDP supervision, assist with habilitation plans, action plans, service documentation, and other required records.
- Assist individuals with accessing community resources, entitlement benefits, housing, food assistance, Medicaid, natural supports, and other resources appropriate to their needs and preferences.
- Work with individuals to identify and strengthen natural support networks consisting of family, friends, and other associates.
- Provide crisis-management and problem-solving support as needed and promptly report crisis situations and follow-up needs to the QIDP/Division Director.
- Administer medications or support medication management when assigned, trained, and authorized, and maintain accurate health-related records as required.
- Monitor the effectiveness and quality of services and communicate concerns to the QIDP/Division Director to support compliance with internal quality standards and regulatory requirements.
- Maintain accurate and timely progress notes, service documentation, incident reports, home visit documentation, and other required paperwork.
- Maintain confidentiality, appropriate professional boundaries, ethical standards, and adherence to Call To Compassion policies, procedures, and applicable regulatory requirements.
- Participate in required training and professional development opportunities to maintain and enhance knowledge and skills.
- Perform additional duties related to individual support, service coordination, and program operations as assigned within the scope of the position.
Requirements
- Bachelor’s Degree in a Health and Human Services related field required.
- Must pass required criminal background check.
- Must possess and maintain a current Nevada driver’s license, automobile insurance, clean driving record, and reliable transportation.
- Intermediate proficiency with computer applications such as Word, Excel, electronic documentation systems, and other PC-based programs.
- Excellent organizational, communication, interpersonal, and time-management skills.
- Ability to multi-task, work independently, collaborate as part of a team, and remain flexible amid changing priorities and workflows.
- Commitment to person-centered, culturally competent services and the ability to maintain appropriate professional boundaries and confidentiality.
- Ability to complete accurate documentation and required reports in a timely manner.
Physical Requirements:
- Ability to lift and carry approximately 25-75 pounds as required by individual support needs.
- Capability to assist individuals with mobility challenges and other activities of daily living.
- Stamina to perform various physical tasks and community-based activities., * Bachelor’s (Required)
Experience:
- working with individuals with intellectual disabiltiies: 1 year (Preferred)
