Job description

Love Never Fails (LNF) seeks a Mobile Clinical Case Manager to join our team. LNF is a 501(c)(3) nonprofit organization with a mission of being dedicated to the restoration, education and protection of those involved or at risk of becoming involved in domestic human trafficking. We are devoted to expressing to each person that they are valued, precious and loved by their community. Our approach is to combat human trafficking through safe housing, workforce development and prevention education., * Provide individual outreach to clients in the Bay Area to provide support on an ongoing basis. Support will be provided in a mobile location such as a park, school, office, coffee shop, etc.

  • Administer comprehensive mental health screening and needs assessments

  • Collaborate with agencies to find long term safe placement if needed.

  • Collaborate with Partners to secure resources clients need

  • Provide engagement activities to assist clients in accepting services to include housing placement, drug and alcohol treatment, mental health services, legal services, and job placement.

  • Determine the reasons for the client instability and use motivational interviewing to support

  • Address client immediate needs by providing information, basic necessities, or referrals to external providers with Behavioral Health, Public Health, and providing psycho-education.

  • Attend the LNF Multi-Disciplinary Team (HT MDT) on behalf of client.

  • Prepare a safety plan in collaboration with partners and clients.

  • Work with clients and their support network to develop a plan for stabilization; ensure they have a safe environment to meet their emotional and health needs.

  • Provide logistical support to the client, linking client to appropriate services (e.g., transportation service needs).

  • Work with LNF Service Management to create a list of local housing placement and services options for your client base in East Palo Alto and Tri Counties area.

  • Crisis intervention, including, but not limited to, development and support for safety plans and meeting the client’s immediate emotional and physical health and basic needs.

  • Advocacy and support for the client with law enforcement, partners and referral agencies.

Confidentiality and Security Protocols

  • LNF Mobile Clinical Case Manager must adhere to strict LNF confidentiality and safety guidelines.

Requirements

  • Clinical - Hands on experience with survivors of trauma, drug and alcohol addiction, and human trafficking, domestic violence, and homelessness are a must.

  • Communication - Professional, timely, and proactive communication with LNF program members, leadership, service providers, volunteers, and partnering agencies.

  • Planning and Organization - Ability to create processes and structures that are easily followed and beneficial to LNF program members, service providers and volunteers.

  • Problem Solving - Ability to use problem solving techniques to resolve issues independently.

  • Self-Management /Initiative/ Learning - Ability to drive the creation and execution of restorative programs with minimal support/engagement from LNF leadership.

  • Technology - Proficient use of computers, Internet, communication devices, and case management databases.

  • Integrity - Strict adherence to LNF confidentiality and safety guidelines. Must have good boundaries and sound judgment.

  • Flexible - Must be willing to be available after hours in the event of a client crisis and respond to emergencies in a timely manner.

  • Transportation - Must have a valid driver’s license and be willing to travel in the Bay Area, * MFT or aMFT with Masters or Bachelor’s Degree in Psychology, Social Work or related field + Clinical Case Management Certificate
  • Must be Spanish-Speaker
  • 40 hours of human trafficking training required (must be able to verify), willing to train the right candidate.
  • Minimum 1 year experience with the following:

  • Case Management

  • Conflict Management

  • Psychology

  • Crisis Management

  • Trauma Informed Training

  • Drug & Alcohol Rehabilitation

  • Domestic Violence Rehabilitation

  • Human Trafficking Rehabilitation
