Job description

Facilitates the development and acquisition of all Re-entry services for residents, (incarcerated and released) i.e., housing, employment, transportation, reconnection benefits, healthcare and family reunification. Increases opportunities for inter-system and inter- disciplinary education and training. Leverages community networks to enhance community supervision and increase knowledge of Re-entry policies and programs. Concentrates on community supervision resources and adjust strategies as sustainability is achieved., * Develops and implements ongoing re-entry transition planning.

  • Develops impact panels, impact of crime classes and other programs involving victims designed to convey the harm resulting from crime.
  • Facilitates access to stable housing upon resident’s re-entry to the community.
  • Ensures that individualized, accessible, integrated and effective community-based health and treatment services are available.
  • Facilitates residents sustained engagement in treatment, mental health and supportive health services as required.
  • Consults family and community members regularly to determine their assessment of the re-entering resident’s adjustment and modify supervision strategies accordingly.
  • Recruits volunteers from community and faith-based groups to increase program capacity.
  • Creates a comprehensive workforce system that is integrated, market-driven, accountable, universal and portable.
  • Facilitates public appreciation for re-entry, (i.e., providing support during residents’ incarceration and upon return, makes families and the community stronger, safer and healthier).
  • Performs field duties and responsibilities (home and agency visits, meetings, conferences, workshops) as required.
  • Maintains all required program documentation (databases, spreadsheets, reports, forms, statistics, etc.).Performs related responsibilities as required or directed. *

Requirements

Do you have a valid Driver’s License license?, Do you have experience in CPR?, Do you have a Bachelor’s degree?, * Three (3) years of experience as a social worker, coordinator or related with direct service within organizations meeting the needs of difficult and hard-to-manage populations; AND

  • Bachelor’s degree from an accredited college or university in Business Management, Social Work, Psychology, Sociology, Counseling or related field; OR
  • An equivalent combination of related experience and/or education.
  • Familiarity with community resources necessary for program success including governmental, private or faith-based preferred.PROOF OF EDUCATION, TRAINING AND/OR EXPERIENCE IS REQUIRED. *, * Knowledge of the methods and techniques of counseling and their application.

  • Knowledge of legal methodology and processing cases.
  • Ability to establish meaningful rapport with counselees.
  • Ability to write meaningful reports.
  • Ability to gather and interpret case histories.
  • Ability to work with various support agencies in exchange for information and data.Ability to organize and supervise meaningful activities relevant to patient needs. *

Special Requirements

  • Must pass background check
  • Must possess a valid Tennessee driver’s license or secure one as of date of employment
  • Must be on call and available for various shifts and weekend work.
  • Must be able to be certified to perform CPR.Must participate in annual in-service training and other training programs as required. *
