Access Case Manager

Access Case Manager: (Bradford, VT) We are looking for a talented Case Manager to join our Access Team & provide immediate support to individuals, couples and families, and assist them in gaining access to mental health & substance use treatment and related services. The primary role is:

• to gather information from client and referral sources,
• schedule clinical assessments,
• provide supportive counseling and possibly some emergency services,
• provide case management and referrals to other services,
• coordinate with billing dept. and administrative staff on insurance and payment,
• complete medical record forms and documentation,
• navigate in multiple software systems to ensure communication and record clinical data elements.

This is a Bachelor level position working as a part of our centralized access team & will involve interface with all other agency programs to assist clients in making strong connections to ongoing treatment.
