Job description

  • Patient Assessment: Conduct thorough evaluations of patients’ health, social, and care needs
  • Care Coordination: Facilitate seamless communication between healthcare providers, patients, and families
  • Care Planning: Develop and implement individualized, person-centered care plans
  • Resource Management: Connect patients with community-based services and support programs
  • Monitoring & Follow-Up: Track patient progress, adjust care plans as needed, and conduct follow-up visits or calls
  • Education & Support: Educate patients and families about conditions, treatment plans, and resources
  • Data Management: Maintain accurate documentation in compliance with regulatory standards
  • Collaboration: Work closely with healthcare teams to ensure coordinated and effective care

In this role, you’ll coordinate comprehensive care for patients with complex needs, ensuring they have access to the services and support necessary to improve health outcomes and quality of life. If you’re compassionate, organized, and motivated to make a real difference, this is the perfect opportunity for you., * Minimum 2 years of care management experience

  • CHW (Community Health Worker) training/certification preferred
  • Strong understanding of healthcare systems and community resources
  • Excellent communication, interpersonal, and organizational skills
  • Ability to work independently while collaborating effectively with a team
  • Comfortable using electronic health records and care management software
  • Willingness and ability to travel within Marin County
