Epic Healthy Planet Implementation Analyst

In this role, the analyst serves as a key leader in the implementation and optimization of Epic Healthy Planet for a mid-sized hospital or health system. They partner closely with population health leaders, care management teams, clinical stakeholders, quality departments, and IT leadership to design workflows that support value-based care initiatives, preventative care programs, and improved patient outcomes.

• Lead Healthy Planet workstreams throughout the implementation lifecycle, including planning, design, build, testing, go-live, and post-production support.
• Facilitate requirements-gathering sessions with operational and clinical leaders to translate business needs into Healthy Planet solutions.
• Configure and optimize registries, Compass Rose programs, care gap workflows, risk stratification tools, and quality measures.
• Support HEDIS and population health initiatives by building reporting tools, identifying care gaps, and monitoring quality metrics.
• Collaborate with analytics and quality teams to develop dashboards, Reporting Workbench reports, and population health insights.
• Coordinate system testing, workflow validation, end-user training, and go-live readiness activities.
• Act as the primary Healthy Planet subject matter expert for assigned projects, providing guidance to stakeholders and mentoring junior analysts when needed.

• 3-5+ years of Epic Healthy Planet experience
• 1+ full lifecycle Epic Healthy Planet implementation
• Epic Healthy Planet Certification
• Strong process documentation
• Experience supporting a mid-sized health system or hospital
• Knowledge of value-based care initiatives
• Experience working with, Quality reporting
• Experience gathering requirements and translating business needs into Epic solutions
