Job description
HarmonyCares is seeking a compassionate CHA to join our Healthcare & Medical Services team, providing high-quality home-based care to patients and families in their own homes. As a key member of our multidisciplinary care team, you will deliver direct clinical services, coordinate individualized care plans, and offer education and emotional support that empowers patients to manage their health with dignity and independence. In this role, you will conduct comprehensive in-home assessments, monitor vital signs and symptoms, administer medications and treatments as ordered, and provide skilled interventions such as wound care, catheter care, and chronic disease management. You will collaborate closely with physicians, therapists, social workers, and caregivers to develop and update patient-centered care plans that align with each patient's goals, preferences, and cultural values. Using sound clinical judgment, you will recognize changes in condition, communicate promptly with providers, and adjust care interventions to promote comfort, safety, and optimal outcomes. Documentation will be completed accurately and timely in electronic medical records, ensuring regulatory compliance and continuity of care. Because HarmonyCares specializes in home health, palliative care, and hospice services, you will often support patients with complex or life-limiting illnesses, offering education on symptom management, medication adherence, and home safety, while guiding families through difficult decisions with empathy and respect. You will work with a high degree of autonomy in the field, organizing your daily visits, prioritizing urgent needs, and maintaining clear communication with the office team to coordinate schedules and services. HarmonyCares fosters a collaborative and supportive culture that values compassionate patient care and employee well-being; you will have access to clinical support, ongoing training, and opportunities to grow your skills in home health and palliative care. This position offers flexible scheduling, competitive pay, and the opportunity to make a direct, meaningful impact on patient lives by delivering personalized, holistic care in the comfort of their homes. If you are a clinically skilled, organized, and compassionate clinician who thrives in a field-based role and is committed to patient-centered care, HarmonyCares invites you to join our team and help us provide exceptional home healthcare to the communities we serve. Provide skilled in-home clinical care, including assessments, vital sign monitoring, and symptom management for a diverse patient population. Administer medications and treatments, including wound care, injections, and other physician-ordered interventions in the home setting. Develop, implement, and regularly update individualized care plans in collaboration with physicians and the interdisciplinary care team. Document all patient visits, assessments, interventions, and outcomes accurately and promptly in the electronic medical record. Monitor changes in patient condition, communicate significant findings to providers, and adjust care plans as appropriate. Educate patients and caregivers on disease processes, medications, home safety, and self-care strategies to promote independence and prevent hospitalizations. Coordinate services with therapists, social workers, and other team members to ensure comprehensive, patient-centered care. Support palliative and hospice care goals by focusing on comfort, quality of life, and emotional support for patients and families. Manage a field-based schedule, plan daily home visits efficiently, and maintain regular communication with the HarmonyCares office team. Adhere to all clinical, safety, infection control, and regulatory standards while representing HarmonyCares with professionalism and compassion in the community.
