Case Manager (Bi-Lingual)

• Coordinate services for individuals enrolled in the HCS and Child-Specific programs.
• Conduct client assessments and assist with the development and monitoring of individualized service plans.
• Serve as a liaison between clients, families, providers, and community resources.
• Maintain accurate documentation and case records.
• Ensure compliance with program requirements, state regulations, and agency policies.
• Advocate for clients while providing exceptional customer service and support.
• Manage office operations and coordinate staff to ensure efficient daily business functions.

Our client, a home healthcare agency conveniently located near Richmond Avenue and Wilcrest Drive in Houston, is seeking a compassionate and organized Part-Time Bilingual Case Manager to support its Home and Community-based Services (HCS) and Child-Specific programs.

Location: Houston, TX (Richmond & Wilcrest area)

Schedule: Part-time, at least 3 days per week (Monday-Friday), * Bilingual (required)

• Experience in case management, home health, HCS, IDD, or community-based healthcare preferred.
• Strong communication, organizational, and interpersonal skills.
• Proficient with Microsoft Office and electronic documentation systems.
• Professional, dependable, detail-oriented, and committed to quality client care.
