Job description

  • Provide direct counseling, case management, and recovery support to individuals receiving structured residential treatment within Clinical Stabilization Services (CSS).
  • Manage an assigned caseload, meet with individuals at the frequency required by their level of care, and build therapeutic relationships that promote stabilization, recovery, and independence.
  • Complete admission and discharge documentation and develop individualized assessments, treatment plans, safety plans, relapse-prevention strategies, and related service documentation with individuals and the treatment team.
  • Coordinate continuing-care and discharge planning, connecting individuals with outpatient treatment, medication services, recovery supports, housing, healthcare, benefits, and other community resources.
  • Facilitate psychoeducational, recovery-focused, and family groups and document participation, progress, and services accurately in the electronic health record.
  • Participate in multidisciplinary treatment-team meetings and collaborate with clinical, nursing, recovery-support, and program leadership to ensure integrated, high-quality care.
  • Monitor program activities and support individuals in following treatment and safety expectations through respectful, trauma-informed, person-centered approaches.
  • Respond appropriately to critical incidents, behavioral or medical concerns, and emergencies; use on-call support when needed and promptly report safety or environmental concerns.
  • Maintain complete, accurate, and timely electronic health records, incident reports, transportation requests, and other required program documentation.
  • Protect confidentiality under HIPAA and 42 CFR Part 2, follow licensing and funding requirements, complete required training and supervision, support transportation as needed, and perform other assigned duties.

Requirements

  • High school diploma or GED required; associate degree or higher preferred.
  • At least one to three years of experience working in a substance-use treatment environment required.
  • Knowledge of substance-use disorders, withdrawal and stabilization needs, recovery-oriented care, case management, treatment planning, relapse-prevention support, and community resources.
  • Commitment to empowerment, personal growth, human rights, dignity, safety, and community integration for individuals served.
  • Two years of sustained recovery preferred for candidates drawing on personal recovery experience.
  • Strong interpersonal, group-facilitation, verbal, and written communication skills, including the ability to respond professionally during stressful and high-acuity situations.
  • Strong organization, time-management, documentation, and follow-through skills, with the ability to manage multiple priorities and urgent needs.
  • Familiarity with internet, email, word-processing, agency communication, and electronic health record systems.
  • Valid driver’s license required, along with the ability to complete required driving training and support transportation when assigned.
  • Ability to work the assigned essential-staff schedule in a 24-hour residential program and meet the physical requirements of the role, including lifting or carrying up to 25 pounds, with or without reasonable accommodation.
